Concierge Medicine at ICM is a tiered care model designed to give you more — more time with your provider, more access, and more support for the aspects of your health that fall outside traditional insurance coverage.

We offer three enrollment tiers: Signature Care, Premier Care, and Per Visit. Each tier provides a unique bundle of benefits, including increased provider access and exclusive services not typically covered by insurance.

Insurance still works the same way. We continue to bill your insurance for covered services. Concierge Medicine enrollment fees are separate — you remain responsible for copays, deductibles, and co-insurance per your plan.

Getting started with Concierge Medicine at ICM is straightforward. Here's what to expect from your first visit through enrollment.

Choose Your Concierge Tier

After your second appointment, you'll have the opportunity to select your Concierge tier. Take time to evaluate your needs and discuss your options with your provider to choose the tier that best supports your care plan.

Flexible Pay-As-You-Go Option

If the tiered approach doesn't meet your needs, a Per Visit model is available. Insurance will be billed for covered visits, but copayments, coinsurance, and deductibles still apply, along with the $75 per-visit fee.

Leaving a Paid Tier

Transitioning to Per Visit Care

If you wish to discontinue your paid Concierge tier and move to Per Visit Care, please review the following before submitting a request.

  • Written request required. All tier cancellations must be submitted using the form to the right.
  • You remain responsible for any outstanding balances and may continue to receive billing notifications through the patient portal.
  • Provider messaging via ChARM is available for 7 days after your last appointment once moved to Per Visit.
  • Per Visit Care does not include ongoing Concierge-tier benefits.
  • A $200 re-enrollment fee may apply to return to a paid tier, unless you re-enroll on the yearly plan.
⚠  Patients of Dr. Karp, Dr. Celmer, or Dr. Klausmeyer: Per Visit Care is currently full for these providers. You must remain on a paid tier or reschedule with another provider.

Yes. ICM maintains an after-hours on-call physician available to all current ICM patients for urgent health matters. This service is for urgent concerns only — please do not use it for routine questions or prescription refills.

Yes. To ensure the high level of service outlined in our Concierge tiers, we limit the number of patients we serve.

Yes. We offer discounts for partners and families enrolled in our Signature Care and Premier Care tiers. Partners can combine their access, and families can add children under 18 at a reduced rate when an adult is enrolled.
